Dear Patient,
Welcome to the Carolina Center for Rheumatology and Arthritis Care. In order to serve you better, we have enclosed an information packet for you to complete. The information in this packet gives detailed information and we ask that you pay particular attention to the bolded sections. Please bring this completed packet with you on youl appointment' Prior to your appointment, the doctor will need to review your pertinent medial records. Please contact any physicians you have seen in the past 12 months (or any rheumatologist you have seen in the past) and have them mail or fax relevant office notesr lab reports, and xrays at least 2 days prior to your appointment. If any authorizations are needed for your visits, you will be responsible for getting them to our ofltce prior to your appointment. If we do not receive you records or authorization, your appointment may be cancelled. You may wish to hand deliver your records and authorrzationto our office to ensure their timely arrival.
Your first appointment with the doctor will consist of 2 parts: first, a review of your medical history and cument complaints through a questiorVanswer session. Second, a complete rheumatologic examination. The first part lasts approximately 25 minutes. The comprehensive exam also will take approximately 25 minutes and includes a thorough evaluation of every joint
for inflammation, range of motion and deformation. Therefore, depending upon the complexity of your problems, you will be spending any where from 45 minutes to t hour with the doctor.
Afterward, if blood work is necessary, it can be done in our office at this time. Expect to be in our office anywhere from 1 to 2 hours on your first visit.
IF YOU ARE UNABLE TO KEEP THIS RESERVED APPOINTMENT, PLEASE NOTIFY OUR OFFICE AT LEAST 48 HOURS IN ADVANCE, SO THAT WE MAY OFFER THE VISIT TO ANOTHER PATIENT. OTHERWISE, YOU MAY BE BILLED FOR
THE DOCTOR'S TIME.
Our office will call you 1 to 3 working days before your appointment to confirm the day and time you are scheduled to be seen. If we are unatrle to confirm with you personally, it will be necessary to cancel your appointment so that the time may be given to someone on our waiting list. If you know you will be unavailable to receive the confirmation call, please call us to confirm vour appointment.
In Charlotte: Our office is located approximat ely .25 miles east of Randolph Road on North Wendover Road; watch for the Park Place Complex. When coming in the front entrance, our office is located in the right atrium. In Rock Hill: 744 Arden Lane is located in the Millwood Business Area off of Herlong Avenue; one-half mile from the intersection of Ebenezer and Herlong.
