Visiting our office is the first step to getting your quality of life back! Please download and complete the new patients forms, and bring them to your appointment, or you may arrive 30 minutes early and complete the paperwork in our office. Please also bring the following to your first visit:

  • Your insurance cards and a photo ID
  • A list of medications with dosage that you are currently taking (including supplements)
  • Results of any related medical or laboratory tests – such as X-Rays, bone density reports, etc.

During your first visit, a nurse will take your vital signs, get your medical history and talk with you about your symptoms. The rheumatologist will then discuss in detail your symptoms and perform a comprehensive musculoskeletal examination. Be sure to explain all the symptoms you are experiencing to your doctor.

Now is the time to ask questions or discuss any concerns with your rheumatologist. We promise to speak candidly with you about your symptoms, diagnosis and treatment options.

Your rheumatologist may order additional lab tests and X-rays. This will help the doctor confirm a diagnosis and formulate a treatment plan for your specific case. Working together, we can achieve a true partnership with one common goal - helping you manage your illness.
